Medication therapy management (MTM) is medical care provided by a pharmacist whose aim is to optimize drug therapy and improve therapeutic outcomes for patients. Beth Vaughan, a pharmacist for over 25 years and an expert in Medication Management, will review your current medications and perform an assessment of potential medication-related problems. She will then create a Medication-related Action Plan (MAP) to monitor progress towards your health goals.
